(ruler-mode-left-fringe-cols): Variable removed.
(ruler-mode-left-fringe-cols): Function replaced by more efficient
implementation `ruler-mode-extra-left-cols'.
(ruler-mode-ruler): Use above new function.  Take into account
that the fringe areas can now be resized.
